Please remember the following guidelines for travel permits:
TRAVEL OUT OF COUNTRY and/or to CANADA:
- Please fax or mail a completed CANADA/UNITED STATES TRAVEL PERMIT signed by Ed Imes, Pacific District Registrar, when your team travels to Canada. Ed only wants this form faxed /mailed to him when a team travels to Canada. See letter from Ed Imes concerning Canadian travel.

- Please fax, mail, or email a completed OSHA TRAVEL PERMIT (attached) when the team travels outside of Oregon OR the country to the OSHA office.
