The following instructions, together with the online instructions and prompts provided by the Acxiom Information Security Services (Acxiom) website, will guide you through the OSHA online background screening process.

1) Read the Notice from the OSHA President on screening and download and read the letter from the District - Dick Coombs.

2) Download and complete the OSHA - APPLICATION/SCREENING DISCLOSURE STATEMENT. The PDF version is linked below. Once read and signed, fax or mail it to the OSHA offices. The fax number to send it to is:  503-293-4031 . The fax is secure only to OSHA personnel. All records are handled with the highest level of confidence, sealed and locked for the season.

3) Download the OSHA - ONLINE BACKGROUND SCREENING PROGRAM INSTRUCTIONS. The PDF version is linked below as well. Once you have read the document, sign in to the online system (Acxiom) and compete the screens per the instructions. The site is located at http://www.mybackgroundreport.com

4) Monitor the Screened Cleared list to insure that your name appears once you are cleared. It is your responsibility to gain clearance, not OSHA or your Affiliate.
